DATA SUBJECT APPLICATION FORM

(REGARDING THE APPLICATIONS TO BE MADE BY THE DATA SUBJECT TO THE DATA CONTROLLER PURSUANT TO THE PERSONAL DATA PROTECTION LAW NO. 6698)

OVERVIEW

Real persons who are defined as data subjects in the Law on the Protection of Personal Data No.6698 ("PDP Law") ("Hereinafter referred to as the "Applicant"), under article 11 of the PDP Law, are granted the right to make claims on processing of personal data.

In accordance with the first paragraph of Article 13 of the PDP Law; the applications to be made regarding the application of the Law to our Company, who is the data controller, must be submitted to us by the following methods.

In this context, the applications to be made to our Company in "written" form must be printed out after filling and signing this form and delivered via;

• Application of the applicant in person,
• Notary,
• Sending it with a secure electronic signature, mobile signature or registered e-mail address to the company’s registered e-mail address info@modaselvim.com or modaselvim@hs01.kep.tr,
• Sending it to info@modaselvim.com or modaselvim@hs01.kep.tr via the e-mail previously stated by you to our Company and registered in our system.

You may be contacted if additional information is required within the scope of the evaluation of your application. Your application will be concluded free of charge, and if additional cost is required, please be advised that a fee will be charged in the amount determined under the relevant legislation.

The information is given on how to deliver the written applications, specific to the written application methods.

The applications submitted to our side will be replied as soon as possible and within thirty days at the latest from the delivery date of the application, in accordance with the second paragraph of Article 13 of the PDP Law. Replies to the applications will be delivered in written form or electronic media in accordance with the provision of Article 13 of the PDP Law.

Your personal data (name, surname, signature, TR ID number, address, telephone number, the applicant's relationship with the Company), which is collected in the act of filling out our application forms in printed form or electronic form, is processed by our Company, who have the title of data controller, in order to exercise your rights enumerated in Article 11 of the Law for applying our Company.

Your personal data can be relayed to the Personal Data Protection Board in order to provide information to authorized persons, institutions and organizations, to follow and carry out legal affairs and to carry out activities in accordance with the legislation.

The legal reason for processing your data is "expressly provided for by the laws and legal obligation" enumerated in Article 5 of the Law, and the legal basis is the the Communiqué on Procedures & Principles of the Application to Data Controllers.

This application form has been prepared to determine your relationship with our Company, if any, to determine your personal data processed by our Company, and to respond to your relevant application within the right and legal time. In order to eliminate legal risks that may arise from illegal and unfair data sharing and particularly, to ensure the security of your personal data, our Company reserves the right to request additional documents and information (copy of identity card or driver's license, etc.) for determination of identification and authorization. In the case that the information regarding your applications submitted within the scope of the form is not accurate and up-to-date or in case of an unauthorized application, our Company does not accept any responsibility for requests arising from such false information or unauthorized application.
